Dear Patients of AllCare Medical Clinic:

This information informs you that AllCare Medical Clinic has closed its medical office effective June 6, 2025. We no longer provide medical care or renew prescriptions. This has not been an easy decision for us. Caring for our community of patients has been a great source of satisfaction and pleasure for us over the years. 

Please arrange medical care with a New Provider at your earliest convenience. You must sign a medical release with your new provider so that we may forward your medical records to your new provider when they request them.

Important Information:

• Until Sunday, June 29, 2025

Contact your new provider and let them know that you would like to establish medical care with them. You will need to sign a medical release with your new provider so that they can fax the signed medical release to us on your behalf. The medical release allows us to send your medical records to your new provider. Your new provider should fax the request to (731-635-6010) so that we may transfer your medical records to them.

• Beginning Monday, June 30, 2025 

Desert River Solutions (DRS) will become the custodian of AllCare Medical Clinic’s medical records so you may contact them through the information below. Be sure to indicate on the form whether you would like your records sent directly to you or to your new provider.
